HEALING MECHANICS PRIVACY POLICY

This notice describes how your medical and personal information may be used and disclosed by Healing Mechanics (the "Company" or "we") and how you can get access to the information.

Primary Treatment: Your health information may be used or disclosed to other health care professionals with your consent for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment.

Health Care Operations:  Your health information may be used to support day-to-day activities and management of the practice as necessary.

Law Enforcement: We may disclose your health care information to a law enforcement official for purposes such as identifying or locating a suspect or missing person, complying with a court order or subpoena, and reporting neglect, abuse, and domestic violence. It may also be necessary to contact the appropriate authorities in order to prevent a serious and imminent threat to the health or safety of a particular person or to the general public.

Public Health: We may disclose your health information to public health authorities for the purposes related to: preventing, controlling and monitoring disease exposure; injury or disability; and reporting problems with products and reactions to medications.

Public Health Oversight:  We may disclose your health information so that public health officials can monitor, investigate, inspect, or license people working in the health care industry or for government benefits programs.

Emergencies:  We may disclose your health information to notify or assist in notifying a family member or another person responsible for your care about your medical condition or in the event of an emergency or your death.

Appointment Reminders: Your contact information may be used by staff to send you appointment reminders, which may be in the form of a text or voice message to your phone. 

Payment:  Your information may be used to seek payment from your health plan, from other sources, such as an automobile insurer, or from credit card companies that you may use to pay for services.

Individual Rights: You have certain rights under the federal privacy standards. These include the right to:

  • Inspect and copy your health information
  • Have your medical data stored in a secure location with restricted access
  • Request restriction on the use and disclosure of your protected health information
  • Submit corrections to your protected health information
  • Receive confidential communications concerning your medical condition and treatment
  • Receive an accounting of how and to whom your protected health information has been disclosed.
